Is #5 the first 93 "R" sold to the public ?

I would look at the build date of #5 and then compare it to #4 and see if there is a large gap between the numbers. This might tell you something about it. At least thats the way the regular preproduction Cobras are. (pre #19)

What else can you tell by the vin ? Can you tell if it's a real "R" by the vin ?

all of the R models are listed in the registry , so you can look up their vin numbers, the vin is the same with only the last 6 numbers being different.

a high mileage 93 "R" would be in the upper 30's low 40's for my guess...

Had offer of 34K for mine has 42Kmiles on it, turned it Down, only 107 Built think I am going to hold on to it with 4993, Still trying to buy first 93 Avaliable to public, rough car though. it is #19 just like my 93R is #19 Wierd as hell I think.
